Introduction Questionnaire

We are deeply grateful for your introductions and continued support in safeguarding the integrity, reputation, and public standing of the Institution, and in contributing to the continued success of its efforts to advance socioeconomic prosperity and positive impact around the world.

We understand that individuals and organizations you introduce, recommend, nominate, or invite for engagement with the Institution are parties about whom you have a reasonable level of knowledge or familiarity and, as appropriate under the circumstances, have undertaken some reasonable research or inquiry. We of course rely on your good judgment in making such introductions and on your commitment to promptly bring to our attention any information, concerns, or circumstances of which you are aware, or subsequently become aware, that could reasonably be relevant to the Institution’s consideration of the party or could present a legal, reputational, ethical, financial, security, operational, public-perception, or other material concern to the Institution or its community. To this end, we kindly ask that you complete the following short Questionnaire. Thank you very much. 

Council Member Introduction & Reasonable Due Diligence Questionnaire

This questionnaire is to be completed by a party introducing, recommending, nominating, inviting, or otherwise facilitating the consideration of an individual for membership or participation in a United World Leaders Initiative or Council (“IC”).

The purpose of this questionnaire is to assist United World Leaders (“UWL” or the “Institution”) in evaluating proposed participants and to document the reasonable due diligence undertaken by the introducing Representative pursuant to applicable UWL policies and Terms and Conditions.

For example: professional relationship, business relationship, government or diplomatic engagement, academic relationship, philanthropic activity, mutual professional network, personal acquaintance, or other.

C. REASONABLE DUE DILIGENCE UNDERTAKEN

If no, we kindly ask that you conduct this expected reasonable review of publicly available information concerning the individual so you are able to answer the above listed questions. 

D. LEGAL, REGULATORY, REPUTATIONAL AND OTHER CONCERNS

By submitting this questionnaire, I confirm that:

  • I have answered the questions above truthfully and in good faith based on information actually known to me and the reasonable due diligence undertaken by me;
  • I have not knowingly withheld any material information that I believe could reasonably be relevant to UWL’s evaluation of the proposed individual;
  • I understand that my introduction or recommendation does not constitute approval or acceptance of the proposed individual by UWL; and
  • I understand that, pursuant to applicable UWL policies and Terms and Conditions, if I subsequently become aware of credible information or circumstances that could reasonably present a material legal, regulatory, reputational, ethical, financial, security, operational, or other risk to UWL, its members, its ICs, or its institutional reputation, I am expected to promptly disclose such information to UWL in writing.
